To start building your MyLife Health Record, you will need to collect your medical notes from your family doctor and other significant healthcare providers you may have. You may need to give them prior warning so they can organize copies to be made for you. They may be able to advise you on which parts of their patient record are likely to be useful to your future healthcare, or they may be willing to enter the information for you. Physical copies will either be in the form of handwritten notes or printouts. Take your time reading through the notes before entering any information. After reviewing the notes some common threads may become apparent in your medical history. You can navigate the sections of MyLife Health Record using either the left menu or by clicking MyLife Health Record - Update from your Personal Home Page.
